The transition from fee-for-service to value-based payments will be one of the greatest challenges the health care system has faced.  Helping providers achieve success in this transition will require collaboration among providers across the continuum of care, health plans, government agencies, not-for-profit organizations, employers, and services providers.  Combining Presentations and Webinars, the Accountable Care Series addresses:
