Massachusetts is experiencing rapid reform with 5 Pioneer ACOs (including Atrius Health) and 19 Shared Savings ACOs, 4 Community Based Care Transition Projects (CCTPs) CMS funded involving 12 ASAPs, 20 Hospitals, Primary Care Physician practices, SNFs, FQCHCs, and CHHA/VNAs and the passage of the new Chapter 224 Health Reform that sets a statewide healthcare cost goal. In this dynamic environment, Atrius Health and the statewide network of 27 ASAPs (Mass Home Care) are in their 3rd year of on-going collaboration to develop best practices around communication, delivery and management of care to the individuals they jointly serve. Emily Brower of Atrius Health and Amy MacNulty of Community Care Linkages will discuss the strategies, collaboration process and results of the practice sites and population health initiatives that broaden the concept of the "care team" to include home and community-based services.
