Putting “Accountability” in Accountable Care

Verisk Health will present the building blocks for population health management, using its DxCG Intelligence clinical and predictive risk methodology.
A comprehensive clinical profile of the population creates a foundation for organizations that answer the call for accountability in health care delivery.
This session will present predictive models that can be used to (1) create rationale medical budgets, (2) quantify the influence of risk in cost variation, (3) support strategies for care management and wellness, and (4) measure organizational and provider performance.

Learning Objectives:
  • Define population health needs, chronic condition prevalence and wellness opportunities Assess condition prevalence of the covered population, measure the contribution of chronic disease to overall health status and cost, and risk stratify the population to develop health strategies
  • Implement actionable medical management interventions that drive better outcomes Identify and engage high-risk individuals likely to be hospitalized, to present in the ER, or have high annual costs
  • Advance the measurement of provider performance using risk based utilization predictions for admission rates, ER use, imaging and other key services Implement clinically credible provider performance contracts that incent providers to manage complex members and reward them for efficient Pinpoint and benchmark unnecessary utilization, gaps in c are, and other inefficiencies impacting overall outcomes

About the Speaker:

Jo Anne Lutz, Solution Specialist

Jo Anne has more than 30 years of experience with diverse healthcare organizations.  Her knowledge spans predictive risk science, cost and quality measurement, clinical nursing, and strategic planning.

Prior to joining Verisk Health, Jo Anne filled senior positions in the following organizations: Director of Channel Partners at DxCG; Healthcare Market Specialist at AdvanceMed, Director of State Healthcare at Codman Research Group; Director of Strategic Planning at University Health Plan.

Jo Anne also worked as a clinical nurse in major medical centers including Case Western Reserve University Hospital, U.C.L.A, and Maine Medical Center.

She has published research in peer reviewed journals in the fields of healthcare management and law
